When a broker has a change of Firm Name, this form is to be filled to change the same at the board.

Requirements for Change of Firm Name
- Application of Change of Firm Name should be duly filled, dated, and signed by the broker / responsible officer of the firm.
Note: It is also required to change the Name of the Firm at DRE.
